Stir the ricotta and cocoa until they are evenly combined. Add milk a spoonful at a time if the mixture feels too thick. Ricotta can have a naturally grainy texture; that is different from a smooth pudding. If you prefer it smoother and have a suitable blender, blend briefly according to the appliance instructions, then check the texture again.
Add your preferred sweetener gradually, tasting with a clean spoon. A little vanilla can round out the flavor, but it is optional.
